2008 U.S. Junior Team Trials Registration Form

The registration form for the 2008 U.S. Junior Wushu Team Trials is now available. It is in PDF format and requires Adobe Acrobat reader to open. Adobe Acrobat can be downloaded for free here.

If you are staying at the university, you will also need to fill out and send in a General Accommodations Form.
